Prior to scheduling an appointment, please confirm if we are in network with your child's insurance carrier. We also offer self-pay rates if you do not have insurance. Please provide your insurance information prior to your child's visit and also bring the card with you at the time of your visit. If we file your claim, you are responsible for only the co-pay at the time of visit. If we are not a provider for your insurance, if you are not insured or we do not file with your insurance company, you may be responsible for the entire charge at the time of service. Please note that while we have a general list of insurance carriers that we are in network with, it is your responsibility to check if we are in network with your specific plan. We are "in-network" with most all commercial plans and some AHCCCS plans. Please
